How to schedule a Face to Face Examination & Consultation

Step 1: To schedule a Face to Face consultation with one of our physicians, please start by Enrolling online. You will be asked a series of questions including medical information. Important! You should enroll even if we do not yet have a physician in your area so that we may contact you as soon as we have a physician available. Be sure to fill out the all forms completely. The forms cover your personal and contact information, medical questions, and other important information that your doctor will require. Remember, all your private information is kept safe and secure using state of the art SSL encryption technology and is stored in a HIPAA compliant database. Upon completing the online enrollment form, click on the submit button. You will receive an email confirmation stating that your information was received. Step 2: Fax your medical records to 1 (866) 424-5159. We cannot schedule your Face to Face examination with the doctor until we have reviewed and verified your forms including: medical records, HIPAA authorization, and payment authorization! Please provide a clear scan or photocopy of your current drivers license.

Step 3: Call 1 (800) 846-6560 and ask to speak with a plan coordinator to schedule your initial appointment. Our office hours by phone are 7:30 a.m. to 3:30 p.m. MST. NNCIP is committed to providing our members with a convenient process for consulting with chronic pain physicians and we request that you arrive for your appointment at least 15 minutes early. We understand that unforeseen events can happen, so if you have a change in availability, please notify your plan coordinator by calling: 1 (800) 846-6560 at least 24 hours prior to your scheduled appointment so that your consultation may be rescheduled.

We WILL NOT schedule your physician Face to Face consultation until we have received all required documents including your HIPAA authorization, reviewed medical records and Photo ID. Your doctor may also review your medical records prior to your scheduled appointment. In some cases, the doctors review may result in a request for more records or re-scheduling or in some cases a cancellation of your appointment. Should this occur, your plan coordinator or your doctors office will inform you.

When attending your appointment with the NNCIP Physician, please bring your original medical records, current/previous medication bottles with you for the doctor to review. Please show up at least 15 minutes before your appointment to fill out any additional forms.

Things you should know:

  1. We will process your authorized payment 24 to 48 hours prior to your appointment, the fee is typically $299 for the initial Face to Face exam/evaluation and $125 per month for one of the following consultations: In person follow-up, phone consultation, or re-script review request.

  2. NNCIP is not a pharmacy, you CANNOT order any medications from NNCIP.

  3. Your documents are kept private and we comply with HIPAA privacy rules. Only your doctor and plan coordinator will have access to the information you provide and your medical records.

  4. As you prepare for your initial appointment and for every subsequent consultation, please be sure to be completely forthcoming with your doctor. Let them know ALL your concerns, previous & current medications including over the counter products.

  5. New State and Federal programs exist today that routinely monitor your prescriptions of any controlled substances. Pharmacies may notify your doctor if they determine that you have recently or are currently receiving the same or similar medications from any other doctor or pharmacy.

  6. Please be sure to TELL YOUR DOCTOR if you have received ANY amount of controlled substances, e.g. pain and anxiety medication within the last 90 days even if you do not think it matters, it usually does...

If you are approved and physical medications are part of your treatment plan, then you must schedule your consultations by logging into your member portal at the top of this page. The consultation plan shown below is an example only, and may not reflect the actual plan your doctor creates for you. Your consult types may be different than shown below. Please remember that the doctor has full discretion as to what type and if any medication is prescribed and NNCIP has absolutely no influence otherwise! Please understand that we cannot guarantee any specific outcome, that's between you and the doctor.

For security reasons, any prescriptions that you doctor may issue are usually sent directly to an approved pharmacy of your choosing. They are either faxed in, called in, or mailed via traceable method directly to the pharmacy. You may elect to either pickup your prescriptions or have them sent to you via traceable shipping method, however shipping charges are your responsibility. NNCIP does not ship any medications.

Monitoring your shipment, Fed Ex will not deliver to PO boxes. Fed Ex requires an adult signature on delivery from a person 18 years of age or older. Once you have opened the Fed Ex package it is important to read the enclosed prescription information sheets that are included with each prescription. The enclosed medication has been specifically prescribed by the doctor who examined you and is based on the information conveyed in your medical records and during the consultation. Our healthcare providers do not assume any risk that results from the use and/or abuse of a medication prescribed by our physicians.
