How to schedule a Face to Face Consultation

Step 1: To schedule a consultation with one of our physicians, please start by clicking on the "Schedule Consultation" button located above. You will be asked to register online and answer a series of questions and provide medical information. Important! Register even if we do not yet have a physician in your area. so that we may contact you as soon as we have a physician available

Step 2: Be sure to fill out the consultation form completely. This form covers your personal and contact information, medical questions, and your preferred method of shipment. Remember, all your private information is kept safe and secure using state of the art SSL encryption technology. Upon completing the registration form, click on the submit button. You will receive an email confirmation from the website stating that your information has been received.

Step 3: Fax your medical records to 1 (866) 424-5159. We cannot schedule your Face to Face consultation with a physician until we can review and verify your medical records and have payment authorization! Along with your medical records, please fax a photocopy of your drivers license, passport or government I.D.

Step 4: Once we have reviewed all the required information, you will receive an e-mail informing you of the time and location of your Face to Face Consultation with our physician. All times are expressed in Eastern Standard Time. NNCIP is committed to providing our patients with a convenient process for consulting with our physicians and we request that you arrive for your appointment at the assigned time. If you have a change in availability, please notify our Customer Service Department at 1 (800) 846-6560 so that your consultation may be rescheduled.

Note: We WILL NOT schedule a physician face to face consultation until we have received your medical records and Photo I.D.

When attending your appointment with the assigned Physician, please bring in your original medical records for the doctor to review. Please show up at least 20 minutes before your appointment to fill out any necessary information.

Upon a successful diagnosis by our physician we will:

  1. Charge your credit card or send COD for the cost of the Consultation Plan which you picked.
  2. Consultation Plan - $299 down for the initial Face to Face and $119 per month and the treatment plan is sent to you. Your prescription
  3. will be mailed to you to fill at a pharmacy of your choosing.

Schedule your remaining refills and phone consultations based on a yearly schedule as follows:

  • Month-1 Refill
  • Month-2 Refill
  • Month-3 Phone Consult
  • Month-4 Refill
  • Month-5 Refill
  • Month-6 Phone Consult
  • Month-7 Refill
  • Month-8 Refill
  • Month-9 Phone Consult
  • Month-10 Refill
  • Month-11 Refill
  • Month-12 Yearly Face-to-Face

Please remember that the doctor has full discretion as to what and if medication is prescribed and NNCIP has no influence otherwise.

Monitoring your shipment, FedEx will not deliver to PO boxes. FedEx requires an adult signature on delivery from a person 18 years of age or older. Once you have opened the FedEx package it is important to read the enclosed prescription information sheets that are included with each prescription. The enclosed medication has been specifically prescribed based on the information conveyed in your medical records and the physician consultation. Our healthcare providers do not assume any risk that results from the use and/or abuse of a medication prescribed by our physicians. Please review the TERMS OF AGREEMENT for more details.
